RELEASE OF CLAIMS AND HOLD HARMLESS
FOR FUTURE ACCIDENTAL INJURIES OR DEATH

I hereby authorize the staff of RHEMA Bible Church to act for me according to their judgement requiring medical attention, and I hereby waive any and all claims for myself and my child against the sponsors of the RHEMA Summer Basketball Camp, and each of their agents, servants, officers, and employers for injury or illness which may directly or indirectly result from participation in the camp. I agree to indemnify each of said parties against all liability or loss, cost, injury, or damage to persons or property which may arise by virtue of the undersigned engaging in the RHEMA Summer Basketball Camp. I further state that my child is in proper physical condition to participate.
